DEFINITION: The active ingredient of Motimin is domperidone. It furthers gastric emptying and has potent antiemetic properties. Motimin has no effect on the gastric secretions.
MECHANISM OF ACTION: The mechanism of action of Motimin is probably on its antidopamine activity. Motimin has virtually no effect of the dopaminergic receptors of the brain so that psychotropic or neurologic effect are practically absent. Motimin is well absorbed and rapidly metabolised.
Onset of Action: Within 1 hour.
Duration of Action: 7-8 hours
SIDE EFFECTS: Side effects are virtually non-existent; exceptionally some transient intestinal cramps have been reported. As can be expected with a compound that does not easily cross the blood-brain barrier, psychotropic side-effects such as fatigue or somnolence are not observed. extra pyramidal phenomena are possible but very rare; they are spontaneously and completely reversible as soon as the treatment is stopped. As the pituitary gland is located outside the blood-brain barrier, the antidopaminergic action of Motimin may induce and increase in the plasma prolactin level. In rare cases that hyperprolactinaemia may give rise to neuro-endocrinological phenomena such as galactorrhoea.
CONTRAINDICATION:GI obstruction or perforation, epilepsy, phaeochromocytoma, extra pyramidal symptoms.
SPECIAL PRECAUTIONS: Renal Impairment.

INTERACTION: Digoxin: Oral dose of digoxin needs to be reduced. Aspirin, Paracetamol and oral Diazepam: Increases absorption of these agents. Anticholinergic agents: Antagonize the effects of domperidone. Phenothiazines: CNS depression enhanced by domperidone. Antimuscarinic agents and opioids: Antagonize GI effects of domperidone. Suxamethonium: Neuromuscular blockade enhanced by domperidone.
INDICATIONS: Nausea and vomiting due to any cause e.g. post operatively, drug induced, radiation, uraemia, hepatitis, peptic ulcer, reflux oesophagitis.
DOSAGE AND ADMINISTRATION: Adults: 1 tablet 3 times daily, 15 to 30 min before meals and if necessary at bedtime. Children: 0.3 mg/kg body weight, 2 or 3 times daily before meals and if necessary at bedtime. Remarks: - When results are not satisfactory, the dosage may be doubled. - When antacids or antisecretory agents are used concomitantly, they should be taken after meals and not together with Motimin before meals.
